In order to maintain the integrity of our care model—which includes full 60-minute appointments tailored to your needs—we have made the difficult decision to transition out of network with private insurance plans.

This change is due to ongoing low reimbursement rates that no longer support the level of individualized, expert care we are committed to delivering.

We understand that insurance can be a concern, and we want to make this transition as smooth as possible. To support our clients:

We provide detailed superbills for you to submit to your insurance for potential out-of-network reimbursement
We offer affordable self-pay rates to keep pelvic floor physical therapy accessible to everyone

Initial Evaluation and Treatment (60 minutes): $150.00

Follow-Up Session (60 minutes): $135.00

At Blossom, we strive to make the payment process smooth and transparent for all our patients. We warmly welcome cash payments and, to facilitate reimbursement from your insurance provider, we provide a detailed SUPERBILL. This document contains all necessary information for you to submit to your insurance company, making it easier to seek reimbursement for your therapy sessions. Please note that reimbursement is subject to your specific insurance plan and out-of-network coverage. We recommend consulting with your insurance provider to understand the reimbursement process and any requirements they may have.

Cancellation Policy

At Blossom Physical Therapy and Wellness, we prioritize providing the highest level of care and service to all our patients, which is why we have implemented a 24-hour cancellation policy. This policy requires that any appointment cancellations or rescheduling be made at least 24 hours in advance. This time frame allows us to effectively manage our schedule and accommodate other patients who may benefit from the time slot. If a cancellation occurs less than 24 hours before the scheduled appointment, a fee of $50.00 will be charged. We understand that unexpected circumstances can arise, and we strive to maintain fairness and availability for all our patients. Your understanding and cooperation with this policy are greatly appreciated, as it helps us ensure that every individual receives the care they need in a timely manner.
