Out-of-Network Benefits
If you have a different insurance plan, you may still be able to receive reimbursement through your Out-of-Network (OON) benefits. Many insurance plans include OON coverage, which may reimburse you for a portion of your treatment costs.
After each visit, we will provide you with a detailed superbill (medical invoice) that you can submit directly to your insurance company for potential reimbursement.

A superbill is a medical receipt with all the necessary codes and details your insurance company requires to process an out-of-network claim. You’ll receive one after each session at Blossom PT & Wellness.

How do I submit a superbill?
Submitting a superbill is simple—you send it to your insurance company (via their portal, fax, or mail) just like you would a claim form. They will process it and reimburse you directly if eligible.
